Fee for Service Provider

I am no longer a participating provider for insurance companies.

Payment is due directly from the patient at the time of service. 

Evaluation visit: $205, Follow up visits: $120 (We allow an hour for each visit, although follow up appointment generally run 40 to 50 min)

Medicare, Worker's Compensation, Auto, MediCal, Tricare

We are not not providers for the above plans.  We are not able to bill them.

Medicare will not reimburse for treatment by an out of network physical therapist

Triwest: as of August 2018 I am no longer a participating provider for Tricare. 

(The new credentialling for Tricare in 2018 requires the clinic to be a Medicare provider to be part of the Tricare system.  Since I am not a Medicare provider it is imposible for me to be in-network.  I have truely enjoyed working with Tricare over the years to help new moms and others.)

Billing your own insurance

We are able to provide you with a "super bill" at the time of your treatment.  The super bill will contain the codes and numbers usually requested. You can use this to submit your claim to your insurance.  Most insurances have a "claim form" that is required along with the information provided on our super bill. You will need to check with your specific plan to determine what your out of network benefits are.

In most cases you will need to download a claim form from your insurance carrier's web site to submit with the super bill.
