I am a participating provider for the following insurance carriers:

  • Coastal TPA and Community Health Plan
  • Guardian
  • Triwest
  • Blue Shield

Billing&Fees

I accept cash, check and charge (Visa, Mastercard, Discover, Debit) at the time of service. My fees are based on time spent with you and the treatments performed during your appointment.  The fee ranges are as follows:

  • $150.00 for the initial evaluation
  • $100.00 for 45-60 minutes follow up appointments
  • $75.00 for visits under 45 minutes
  • $25.00 "no-show" fee for a scheduled appointment

Supplies:

  • Prometheus personal use sensor: $44.00
  • Hollister personal use sensor: $100.00
  • Dilator: $10.00
  • Kinesiotape roll: $12.00

Billing&Special Considerations

For non insurance patients I request payment at the time of treatment.  I am happy to bill your insurance carrier as a courtesy.

Some insurance companies allow for out of network exceptions when there are no "in network" providers in your area.  Check with your insurance carrier to see what "their rules" are.  My letter of specialty may assist you in getting "in-network coverage".

Medicare

I have elected not to be a Medicare provider.  If you have a secondary insurance you may be able to get some reimbursement for physical therapy.  At the completion of treatment I will submit a cumulative invoice to Medicare, which will be denied.  Using this denial you may be able to bill your secondary insurance.
