I am a participating provider for the following insurance carriers:

• Coastal TPA and Community Health Plan

• Blue Shield

Billing&Fees

• Your insurance will be billed at the time of your visit.  
• If I am out of network with your insurance carrier, payment will be due at the time of your visit.  I will bill your insurance as a courtesy.

Billing&Special Considerations

For non insurance patients I request payment at the time of treatment.  I am happy to bill your insurance carrier as a courtesy.

Some insurance companies allow for out of network exceptions when there are no "in network" providers in your area.  Check with your insurance carrier to see what "their rules" are.  My letter of specialty may assist you in getting "in-network coverage".
